What we do

Community Health works to support people at every age and stage of life to improve health outcomes.  We work within a social model of health that recognises the effect of social factors on the health and wellbeing of individuals and communities. We combine clinical health service delivery and social supports with a focus on prevention and early intervention initiatives for collective health impact. Our aim is to support people in our community – with a priority on populations who typically experience barriers to accessing services - to achieve their goals and optimise their health and wellbeing.

At Western Health the Integrated Community Health Service has 3 primary community health centres offering a wide range of services including allied health, counselling, family violence support, dental and nursing.  We also provide family violence support through our partnership in the Orange Door networks of Brimbank and Melton.

We offer both one to one individualised support and group programs.  Our services run primarily from our service sites however in some services we offer telehealth (phone and video) as well as home-based and outreach services that ensure we reach the people who need us most.

Our dedicated clinicians will work with you to develop an individualised plan based on your personal goals.

The Integrated Community Health Services are broad and wide reaching.  Our aim is to support people in our community to achieve their health goals and enjoy a happier and healthier life.

Community Health programs often incur a fee that is based on an assessment of income. For more information you can access the Community Health Fee Schedule

Through our community health locations, we also offer some National Disability Insurance Scheme (NDIS) and Support at Home services for adults. 

Who this service is for

Community Health provides care for people of all ages.  We prioritise care for those most in need. Some programs require a Health Care Card or Pension Card. If you are not sure about eligibility, please give us a call. 

Our paediatric programs require referrals from a general practitioner or paediatrician.

For people over the age of 65years (50 for Aboriginal and Torres Strait Islanders) you are required to initially check your eligibility with My Aged Care by ringing 1800 200 422.

We strongly believe in and support the rights of all of our clients. When it comes to the delivery of our services, your rights are described in the Australian Charter of Healthcare Rights and the Aged Care Statement of Rights if you receive our support through Aged Care funding. 

The Aged Care Statement of Rights is part of the new Aged Care Act (2024) that outlines the rights of older people accessing government funded aged care services.  It ensures that rights that include independence, autonomy, respect for privacy, safe and quality care as well as the right to raise issues without reprisal are maintained. 

For Aged Care clients, there are also new increased protections for whistleblowers.  This enables older people, their families, carers, representatives and workers to report people or organisations who they believe have not provided a standard of care in line with the aged care laws without fear of reprisal.

Part of the National Disability Insurance Agency which aims to support a better life for hundreds of thousands of Australians with a significant and permanent disability and their families and carers.

The Orange Door
Help for people who are experiencing or using family violence or who need support with the care and wellbeing of children and young people.
