The General Practice Integration Unit strengthens the connection between Western Health and General Practice to enhance patient care.
Our role is to:
Implement quality improvement activities to optimise the quality of inpatient handover and Specialist Clinic correspondence with General Practice.
Provide a central escalation point of contact for General Practices accessing hospital services.
Record, respond to and implement quality improvements based on General Practice feedback.
Provide regular updates to GPs about Western Health services via the General Practice Newsletter.
Provide advice to health service staff on issues relating to General Practice.
Liaise with community partners on projects to improve the Acute/Primary Care interface.
Western Health has transitioned to HealthLink as our Secure Messaging Delivery (SMD) provider for clinical documentation. Since 15 October 2024 we have sent thousands of cilnical documents via HealthLink. Effective from December 2024, Western Health will no longer use PulseNet to send clinical documentation. Please refer to the correspondence dated 18 December 2024 for further information.
While our GPIU can assist you with the transition to receiving clinical documentation via HealthLink, it is important that you take steps as outlined in the correspondence below, to prepare and ensure you can receive clinical correspondence from Western Health.
