Multidisciplinary team including a geriatrician, fracture liaison nurse, accredited exercise physiologist and bone densitometrist, the Falls and Fractures Clinic provides comprehensive assessments and treatment plans for the prevention of falls and fractures in older adults aged over 65 years.
Patients must have:
Aged 65 and over
No serious memory deficits (MMSE >18)
Ability to mobilise independently or with use of gait aid (frame, stick etc)
Provided consent/be willing to attend the clinic
Patients may also present with
Multiple falls (>2 within past 12 months)
Single falls with established gait and/or balance deficit
Unexplained fall with apparent complex medical cause
History of symptomatic or asymptomatic fragility fracture
Clinical or paraclinical (BMD) risk of fractures
Patients from aged care facilities
Wheelchair bound
Dementia/cognitive impairments (MMSE<18)
Aged under 65
Referrals for the Falls and Fractures Clinic must include:
Patient medical history
Recent pathology results (patients sent request if no recent results available)
Medication list
Other relevant information such as existing care plans
Indication if DXA scan is required (DXA can be requested every 2 years)
Please note that the following details are required for referral/attendance at the Falls and Fractures Clinic. Our fracture liaison nurse will follow up with referrals containing missing information.
