Our Residential In-reach Service offers specialty health care assessment and a rapid response service for people with complex medical conditions who reside in Residential Aged Care Facilities (RACF) of out catchment area.
The team comprises of nursing and Medical including Geriatrician, Registrar, and Clinical Nurse Consultants.
The aim of the service is to establish an effective rapport with the RACF (residents, family and staff) to optimize the health and wellbeing to prevent future avoidable hospital presentations/admissions.
The service focuses on:
Building rapport and trust
Providing education and resources
Providing clinical support
As the service hours extend outside of normal business hours it provides staff who work evenings and weekends with an alternative to sending the resident into the Emergency Department.

Upper and Lower Respiratory Tract infection (URTI/LRTI)
Cellulitis
Fever of unknown cause
Falls and potential traumatic injury
Dementia including BPSD
Functional decline
Dehydration
Post-operative infection
Heart failure (CCF)
Pain and other distressing symptoms
Outbreaks (gastroenteritis, Influenza, COVID)
Post hospital discharge follow up (including newly transitioning to residential care)

Referral Process
We accept phone referrals 24 hours 7 days a week.
Nursing staff can visit 7 days a week between the hours of 8am to 9pm, however medical cover is limited to business hours Monday to Friday.
