Hospital in the Home (HITH) provides care in the home that would otherwise need to be given during a stay in hospital. HITH often provides an alternative to admission to hospital, or an opportunity for earlier transfer home, than would otherwise be possible.
Each patient is assessed to work out if HITH can meet their medical needs. If HITH is suitable a plan of care is made in consultation with the team.
Western Health HITH covers 4 Campuses:
Footscray Hospital
Sunshine Hospital
Williamstown Hospital
Sunbury Day Hospital
At Western Health HITH, we aim to:
Provide continuous care from the hospital to the home environment
Coordinate your care from hospital to home
Monitor your progress in consultation with your treating medical team
There is no cost to receive HITH treatment. HITH staff can be contacted at any time during the day or night (24 hours a day, 7 days a week).
As a patient of HITH you remain under the care of the doctors that looked after you when you were in hospital.
