Community Based Rehabilitation (CBR) aims to assist clients and their carers to reach their maximum potential and independence.
Clients are provided an individualised, short-term, goal-orientated therapy program with an emphasis on self-management and patient education.
Both individual and group therapy sessions can be provided depending on the established goals, in conjunction with a therapy program for clients to complete at home.
There is no cost for therapy and Interpreters are used as required.
We provide therapy for client’s who require rehabilitation for:
Musculoskeletal conditions
Neurological conditions
Are falling or have a high risk of falling
Age related changes where there has been a significant acute change in function
Chronic pain (focussed on self-management)
The therapists primarily treat clients at the centre; however limited therapy can occur in their home or a combination of both. The length of time a client will be involved with CBR is discussed with the client and carer taking into consideration their goals and capabilities.

Eligibility
Clients over the age of 16 years
Clients who have achievable rehabilitation goals who require short term multi-disciplinary team involvement.
Clients who are medically stable.
Clients must consent and be motivated to participate in therapy. The client is able to understand the nature of service and is able to and agreeable to participate in therapy (i.e. able to attend multiple therapy sessions, attend during work hours and access our centres)
Resides in the following catchment areas: Cities of Brimbank, Hobsons Bay & Maribyrnong.
The following patients are not treated through CBR:
Eligible for funded therapy - if eligible for TAC / Workcover /DVA funding (for a single discipline only), reside in HLC or receiving services through a high care home care package, private services will be recommended.
Equipment or home modifications – where there are no other rehabilitation goals. Please refer to local Community Health Services for more information.

Monday to Thursday 8AM to 6PM. Friday 8AM to 4.30PM
Location
Rehabilitation is offered on two sites. Clients will attend the centre that is closest to their residence.

CBR is located on the far right of the main hospital entrance. Enter through the glass doors and our reception is on the left.
The Williamstown Hospital
The entrance is located in Stewart Street. Enter through the glass doors and the reception desk is on the right.

Referrals
To refer a client, complete and fax the referral to: Intake. Fax: 8345 6529
Referrals should include:
patient details including address, date of birth and contact phone numbers
the reason for referral
relevant clinical history for the client
list of current medications
any risks to clients or staff
your contact details
Once a referral has been received, one of our staff will phone the client to organise a first appointment or discuss a more appropriate service to meet the client’s needs.
