We provide community-based treatment to older adults in Western Melbourne during an acute phase of mental ill-health.
By providing treatment in the person's home or usual place of residence, we aim to prevent hospitalisation where possible, or minimise the length of an inpatient stay.

We do not replace the need for community nursing services for functions such as medication administration.
Referrals
We accept referrals for this service via WH Acute Inpatient Unit and APATT.
If you would like to discuss treatment or care plans, contact ICT on 9288 7150 and ask to speak to the key clinician, Monday to Friday, 9am to 4.30pm.
