What is the Disability Liaison Service?
The Disability Liaison Service was introduced by the Department of Health as an emergency response during the Covid19 Pandemic in 2020.
The service provides various forms of support for all people who identify as living with disability.
We are here to assist people with a disability, their families and carers to access services across Western Health and other health and human services as required.
This may include:
• Community Outreach Support
• Access to ED and Specialist Clinics
• Inpatient Support
• Assistance with Discharge Planning
Disability Liaison Coordinator's work together with people with a disability and their health care teams to provide safer, accessible and more inclusive care during hospital admissions and outpatient clinic appointments. We aim to improve health outcomes, shorten length of hospital stays, reduce re-admissions and improve overall patient experience for people with a disability.
