Geriatric Medicine Clinics

The Geriatric Medicine clinics are an outpatient service which aims to provide timely access to specialist geriatrician assessment and management of ageing related syndromes.

What we do

  • Assessment and diagnosis of cognitive impairment (where referral does not meet criteria for Cognitive Dementia and Memory Service)
  • Post diagnosis support and management for people living with dementia (e.g. care planning, education)
  • Significant dementia related symptoms (BPSD)
  • Delirium
  • Frail older people who have had multiple re-admissions to hospital
  • Medication complexity or polypharmacy
  • Falls, mobility decline or functional decline requiring assessment (where referral does not meet criteria for Falls and Mobility Clinic or Falls and Fracture Clinic).

Who is this service is for

Inclusion criteria

  • People over the age of 65 or Aboriginal and Torres Strait Islander people aged over 50 years. People under 65 years may be considered if they have a geriatric syndrome (please discuss with lead Community Geriatrician).

Exclusion criteria

  • Clients who are medically unstable and require management within an inpatient hospital ward environment.
  • Routine chronic disease management appropriate for GP care.
  • Patients with primary psychiatric concerns should be referred to the appropriate psychiatric services.
  • People who have recently been assessed by a Geriatrician, and the situation has not changed, are unlikely to need re-assessment. Please discuss with lead clinic Geriatrician.
  • Capacity assessment as single issue.
  • NDIS, VCAT or fitness to drive assessment.

Referrals

Who can refer

The Geriatric Medicine Clinic accepts referrals from Western Health inpatient acute and subacute wards, Emergency Departments (ED) and Western Health at Home community services, GP or specialist.

How to make a referral

Referrals can be made by telephone, fax or mail to the intake on the contact numbers listed below.

The Geriatric Medicine Clinic referral form below should be completed if possible, but a referral letter from a GP or medical specialist will be accepted.
