Hospital in the Home (HITH)

Hospital in the Home (HITH) provides care in the home that would otherwise need to be given during a stay in hospital. HITH often provides an alternative to admission to hospital, or an opportunity for earlier transfer home, than would otherwise be

Each patient is assessed to work out if HITH can meet their medical needs. If HITH is suitable, a plan of care is made in consultation with the team. Western Health HITH covers:

  • Sunshine Hospital
  • Footscray Hospital
  • Williamstown Hospital
  • Melton Health Hub
  • Bacchus Marsh Hospital

     

At Western Health HITH we:

  • Provide continuous care from the hospital to the home environment
  • Coordinate your care from hospital to home
  • Monitor your progress in consultation with our HITH medical team 

There is no cost to receive HITH treatment. HITH staff can be contacted at any time during the day or night (24 hours a day, 7 days a week).

 

We can receive referrals from: 

  • Wards and Emergency Department
  • GP’s
  • Virtual Victoria Emergency Department (VVED)
  • Urgent care centres
  • Other health services

     

Hospital in the Home Common Admission Conditions and Treatments

  • Anticoagulation therapies (clexane and warfarin) for conditions such as Pulmonary Embolus, Deep Vein Thrombosis, Atrial Fibrillation
  • Short term intravenous antibiotics, via intravenous cannula (IVC) or central venous access device (PICC line) or intra peritoneal (for dialysis patients)
  • Long term intravenous antibiotics via central venous access device with a Baxter infusor
  • Acute and complex wound care wound care – Negative pressure wound therapy (VAC machine)

Post operative care for example, care and removal of drain tubes at home.
