HARP is a short term specialist service that supports people with chronic disease and/or complex needs to better understand and manage their health and social needs.
This program aims to prevent avoidable hospital admissions and emergency department presentations by providing an integrated care coordinated approach. This involves:
Assistance to coordinate and manage services and medical appointments
Provision of education specific to your medical condition and strategies to self manage your health
Planning your care with you, your carer and other health clinicians
Linking you into longer term community supports where appropriate.
The Western HARP Program provides services for:
People with chronic heart disease
People with chronic respiratory disease
People with diabetes
Older people with complex needs
People with complex psychosocial needs
People with other chronic diseases and complex co morbidities
Children with asthma and eczema.
Referrals
To be eligible for Western HARP services you must have a defined chronic disease and or complex care need and be at risk of frequent presentations to the emergency department.
Your referral will be reviewed and if it meets our criteria, you will be contacted by a clinician to arrange an assessment.
Referrals can be made by your GP, other health professionals and community agencies. Referrals can be made by completing the referral form and faxing it to 8345 6529.
