The Western Post Acute Care program (PAC) provides health and community care services to people who need support to recover following their stay in a public hospital. The care is provided in the person's home or at another suitable place, for example a family member’s home.
We can see people from 1-2 days to 4 weeks immediately after they go home from hospital.
We see people who live in the area covered by Western Health. This includes the council areas of Brimbank, Hobson’s Bay, Maribyrnong, Melton and Wyndham.
We will organise appropriate services and monitor the person’s progress. When the Post Acute Care service period finishes we will help with the transition onto community services if appropriate and available.
Eligibility
To qualify for the Western PAC program, patients must:
live within the 5 municipalities of the west (Brimbank, Hobsons Bay, Maribyrnong, Melton, Wyndham)
be medically stable and been assessed as safe to be discharged, and
need short term services to assist with recuperation or transition to an identified continuing service.
PAC accepts referrals from acute, subacute and emergency departments of all Victorian public hospitals. Patients from private hospitals are ineligible.
