Eligibility Criteria
Essential criteria for referral to be accepted:
- Aged 65 and over or Aboriginal & Torres Strait Islander aged 55 and over
- Patient does not reside in residential aged care facility
- History of falls (More than one can apply):
- Patient reports multiple falls in the last 12 months
- Patient reports ≥2 falls in the last 6 months
- Single faller with established gait and/or balance deficit
- Unexplained falls with apparent complex medical cause
Additional referral information:
- History of symptomatic or asymptomatic fragility fracture
- Clinical or paraclinical (e.g., low BMD) risk of fractures

Triaging of the referral will determine which service is best suited to your patient's needs.
Please include the following information when referring:
- Current medication list
- Up to date medical history
- Recent hospital discharge summaries (other than Western Health).
- Results of any investigations (including recent pathology, other specialist or allied health reports

Falls and Fractures Clinic (westernhealth.org.au)
The Falls and Fractures Clinic is a multidisciplinary clinic including a geriatrician, a fracture liaison nurse, an accredited exercise physiologist and a bone densitometrists. This clinic provides a comprehensive assessment of an older person risks of falls and fractures. Patients who attend will undergo a comprehensive geriatric assessment and have a care plan developed that will include interventions to prevent the risk of recurrent falls and osteoporotic fractures. After initial assessment, patient will be followed up once (i.e., 6-month follow-up) to monitor their progress.
Falls and Mobility Clinic (westernhealth.org.au)
The Falls and Mobility Clinic provides a Geriatrician only assessment to clients that are falling, and the cause of the falls is unknown. At the conclusion of the assessment the patient & GP is provided with information on appropriate falls prevention plan and recommendations. If indicated and appropriate, referral to Allied Health for assessment and therapy through Community Based Rehabilitation or appropriate local service.
