The Transition Care Program has Bed Based and Community Based programs.
The Bed Based Transition Care Program is a voluntary short-term program which offers people at the end of their hospital stay time to further recover and decide on the best place to live in a home-like environment.
The Community Based Transition Care Program supports people at the end of their hospital stay. We help you to return to the things you want to do at home, with the added support that you might need. We provide this service in a number of different settings:
Bed Based Program which is located in Williamstown Hospital
Dementia Management Unit, Sunshine Hospital, and
Community Based Program is offered to clients in their own home.

Brokered Services (External Services). For example home help, shopping assistance, showering assistance. This is for the Community Based program only.
