The Renal service is dedicated to treating acute and chronic medical and renal conditions to preserve native kidney function. We are also dedicated to improve patient pathways to Renal Replacement Therapies to ensure the best outcomes for our patients.
We are excited to be working in partnership with the Department of Health – Renal health Clinical Network and Aboriginal Health, General Practice Victoria, Macedon Ranges North Western Melbourne Medicare local and Kidney Health Australia on an early detection of CKD project eMAP CKD: VIC, in our local community.
We have also partnered with Kidney Health Australia on 2 public education forums in Sunshine and Werribee as part of the Kidney Health Australia Kidney Examination for You (KEY) programme.
EMAP: CKD VIC – Electronic Management and Assistance in Primary care in Victoria.
Early detection of chronic kidney disease (CKD) and appropriate clinical management of detected cases is the only practical means available to reduce the burden of CKD in the foreseeable future in the Western Health Catchment area. This project is a joint project of Western Health, Macedon Ranges North Western Medicare local, General Practice Victoria & Department of Health.
The specific aim is:
To improve the detection rate of CKD in an 'at risk' population
To Increase the proportion of patients with established CKD who achieve target lifestyle, blood pressure, lipid and glycaemic control and with initiation of reno-protection therapy where indicated.
