Subacute and Nonacute Assessment and Pathways (SNAP) is a service at Western Health which aims to make sure that patients have access to the Right Care, at the Right Time and in the Right Place.
Once a patient is medically able, we talk to them, their families and their treating teams to work out the best service for them to have to be able to try and get back to doing the things that they want to be able to do.
Our staff have specialized skills across Aged Care, Rehabilitation and Nonacute Care and have a passion for meeting patient needs at all stages of their care.
