Subacute and Aged Care Services help people who:
Have an injury, illness, medical condition or disability
Have activities in their everyday life that they cannot do
Would benefit from therapy to try and improve their function
Do not need acute care.
We focus on the person's goals and abilities rather than the underlying medical problems.
We aim to:
Increase a person's independence
Increase a person's ability to do the things they want to be able to do
Reduce a person's long term health care needs
Reduce a person's need for community care
Maximize the effectiveness of services at all stages of a person's hospital care
Subacute and Aged Care Services include:
Western Health has approximately 200 sub-acute beds (as of October 2018). Subacute and Aged Care services are delivered across the Footscray Hospital, Sunshine Hospital, Williamstown Hospital and Hazeldean Transition Care.
