Patients from care facilities

Patients that are attending Adult Specialist Clinics (outpatient) Department from 24-hour care facilities such as nursing homes must have someone come with them to their appointment at the Outpatients Department. This can be a family member or a professional. If the patient attends on their own, they will be sent back to their facility without being medically reviewed. If there are any concerns please phone the Adult Specialist Clinic communication centre on 03 8345 6490.

We make every attempt to make sure that patients from care facilities are seen as quickly as possible but sometimes there can be delays. The below documents will be given to the patient to return to the care facility:

• Nursing transfer letter. This form is required to be filled in by the care facility so that all relevant information for the patient is received at their next visit.

• Patient Facility Correspondence. This is to inform the care facility of current and future required treatment.

These documents help us to make sure that we are providing the best care possible. Staff will assist with any questions or concerns you may have.
