At the BASICS, we provide support to consumers who have a mental health concern or dementia and live in a residential care facility. We also support their family and carers.

Who can use our service?
Individuals are eligible for our service if they:
Are aged 65 years or over, or younger consumers with age-related mental ill-health
Have disturbances of thoughts, mood or perception
Have behavioural or psychological symptoms of dementia
Live in a residential aged care facility
Are being discharged from an Aged Persons Acute Inpatient Unit to a residential aged care facility
Are transitioning from an acute hospital admission into a residential aged care facility
Any person living in a residential aged care facility in the MidWest/Southwest area of Melbourne can be referred to us.

How we work
What happens after referral?
After referral, one of our mental health clinicians will:
Visit the person in their current care environment
Interview and assess their wellbeing and mental health status
Consult with the person, their family, carer or GP to develop a plan that takes into account the consumer's preferences, wellbeing and wider social and cultural background
Visit to offer support and further reviews as needed
Our team
Our team is staffed with a diverse range of highly skilled mental health clinicians. Our clinicians complete thorough individual assessments to complement care planning that will help individuals and their carers through the individual's recovery journey.
General practitioners (GPs) remain the primary health care professional. We liaise with GPs throughout the individual’s time with us.

We accept referrals for this service via centralised triage.

If you would like to discuss treatment or care plans, you can contact the BASICS on 9288 7150 directly and ask to speak to the key clinician, Monday to Friday, 9am to 4.30pm.
For out of hours support, call Triage. If your consumer is presenting in an emergency situation, call 000 for an immediate response.
