What we do
The aim of the HOPE program is to support people for a period of 12 weeks, providing assertive and coordinated care post-discharge, via both outreach and clinic-based therapeutic interventions to reduce the risk of suicide re-attempts and deaths by suicide.
Keeping with the goals of the Victorian suicide prevention framework 2016–25, the Sunshine HOPE team aims to build resilience in individuals who have made a suicide attempt by providing intensive and individualised therapeutic care and psychosocial support.

Who can use our service:
The HOPE program was developed to provide support to individuals discharged from Emergency Departments, medical wards or inpatient units:
Following a suicide attempt, or
Having displayed suicide ideation, intent or planning
Who we are:
The HOPE team is a multidisciplinary team based in Sunshine.
Each consumer is allocated a Psychosocial Support Worker and Mental Health Clinician to deliver practical support and evidence-based therapeutic interventions to address the unique, economic, historical and environmental factors that have contributed to, or continue to contribute to, a person’s suicidality.
